What you’ll do:
- Provide advanced ADL care, incontinence management, and safe transfers
- Assist with hospice support and report changes in client condition
- Monitor memory-care safety and implement fall-prevention strategies
- Support physical therapy exercises and mobility plans
- Complete accurate documentation through smartphone time tracking
- Communicate effectively with the care team and family members

Requirements:
