You’ll work with individualized care plans, collaborate with families and clinicians, and be part of a team that values skill development and professional growth. Shift availability and hours may vary based on client care needs and caregiver availability.
What you’ll do:
- Provide hands-on ADL support including bathing, grooming, and incontinence care
- Assist with safe mobility and therapy exercises as directed by care plans
- Monitor memory-care safety and prevent hazards during overnight hours
- Drive clients to appointments and run errands when needed
- Provide medication reminders and report changes in condition
- Support hospice-related tasks and communicate with the interdisciplinary team
- Complete smartphone time tracking and documentation per agency guidelines

- Pass criminal background check
